AGMS Scheduling Appointment Booking Form (8th Grade-Ms. King)

Welcome, Azalea Gardens Middle School 8th-grade students and families! My name is Ms. V. King, and I am the school guidance counselor. I am pleased to offer this opportunity for you to schedule a meeting with me to discuss your student’s high school credits and preparation for the transition to high school.

Appointments for this event will be available from October 26, 2026, through January 15, 2027, on Mondays, Tuesdays, and Fridays. Please complete one form for each student you would like to discuss and select the appointment date and time that work best for your family. Be sure to include a current address, email address, or phone number so that the guidance office may contact you to confirm your appointment. To protect student privacy, please do not include detailed confidential information in this form.
