2026 Summer Child Care Form
YOU MUST BE A CURRENT CCS EMPLOYEE TO TAKE ADVANTAGE OF THIS PROGRAM

COMPLETE ONE FORM PER CHILD
  • Child Care Hours:  8:00 am-12:00 pm, Monday-Friday
  • First Day: Wednesday, May 27th
  • Last Day: Friday, July 24th
  • Closed June 19th for Juneteenth Holiday & July 3rd for 4th of July Holiday
  • Location: Carmel Wellness Center, 515 East Main Street
  • Ages:  Infant to 12 years old
  • Cost:  $13.00 for childern 3 years and older, $14.00 for infants up to 2 years of age
  • Payments will be deducted at the end of the summer through payroll deduction  
  • More information regarding dedcution will come at the end of the summer
Particpants must be signed up 48 hours in advance.  Once a spot is committed you will be charged.  No refunds or cancellations.  No make-up days or switching days will be allowed.  Only 25 childen will be accepted per day.
  • No Food or Toys alllowed
  • Please have infants in a clean diaper and have chidren use the restroom prior to arrival.
